CWSDPMI may reboot on certain hardware/DOS configurations

Summary:

On certain hardware/DOS configurations, CWSDPMI will reboot on the first
mode switch back to real mode from protected mode.  Changing the size
of the environment will allow the application to work.

Analysis:

Due to a Pentium III bug (E67).  Thanks to Symantec for the analysis.
A serializing jump in MSWITCH around line 191 fixes the problem.

Workarounds:

Change environment size by at least 16 bytes (add a dummy environment
variable).  Use the 2008 r5 refresh, or r6 test release.
